WAIVER FORM *COVID-19: UNDERAGE PLAYER 2023 & COPYRIGHT

RELEASE, INDEMNIFICATION PARTICIPATION AGREEMENT & REFUND POLICY

*** I AGREE THAT NO REFUNDS shall be given for any reason to participants who CANCEL or fail to show up for any camp/event that Canadian Edge LLC is hosting or participating in. ***

 

As the parent/legal guardian, of a player under the age of 18 years participating in any CANADIAN EDGE LLC training session, and/or event, including all non USA Hockey sanctioned tournaments held during the 2023-2024 year. I understand that neither I nor the participant I have registered are covered under any protection plan or, other supplementary insurance plan. I understand the risks associated with training during the COVID-19 Pandemic.

 

In consideration of being permitted to participate, I myself, my heirs, executors, administrators, successors and assigns do hereby remise, release and forever discharge, waive and save harmless, protect and keep indemnified CANADIAN EDGE LLC and the owners/operators/sponsors of these session(s) with which I participate/ am registered with.

 

Canadian Edge LLC representatives have informed me of the importance of having additional liability insurance in the event of an on-ice incident in which I may be named as being negligent.

 

I hereby acknowledge that I understand the risk of being injured while playing the game of ice hockey.

 

I also give Canadian Edge LLC and it’s partner’s permission to take videos and photographs of me that could be used for future marketing/promotions via internet and television.
